At Remembrance Hospice, our hospice care team includes a variety of dedicated experts that are specifically focused on improving the quality of life of our patients and their families by addressing their physical, emotional, and spiritual needs.
Our care professionals collaborate with the patient, their family, and their healthcare provider to develop a personalized care plan to address their unique needs and reduce hospitalizations and enhance the quality of life of our patients. The team includes the following:
- Attending Physicians: Our physicians are experts in pain and symptom management due to life-limiting illnesses. They are always available for consultation and to address symptoms.
- Hospice Aides: Remembrance Hospice utilizes State Tested Nursing Aides (STNA’s) to support our patients and guide their families in providing care to the patient such as executing Activities of Daily Living.
- Social Workers: Our social workers help address the emotional needs and the overall well-being of our patients and their family. They assist patients with placement needs, and much more.
- Nurses: Our Registered Nurses and Licensed Vocational Nurse will perform a variety of roles including home visitations.

David Noggle CEO David is the CEO of Remembrance Hospice. He brings 20+ years of experience in patient focused healthcare. He was born and raised in Toledo, Ohio. He has owned and operated several Home Health and Hospices where the patients always come first. “It is our goal to create a patient centered experience for everyone at Remembrance Hospice. The Patient always come first and by doing this, we can help a family and the patient make their finally journey with dignity, love and compassion.”

Tonya Micheli, LPN, CDPCorporate Director of Business Development Tonya is the Corporate Director of Business Development for Remembrance Hospice. She comes to us with over 30 years experience as a Nurse, with 20 of those years in Business Development in various levels of care. From inpatient rehab to home health, Tonya has successfully led dynamic marketing teams, driving performance and spearheading revenue growth initiatives while keeping the needs of the patient the primary focus. “It is an honor to work with the hospice/palliative population to help the patients, and their families navigate a very difficult time in their lives”

Amber Toot, RN Clinical Director Amber has been a nurse for 14 years. She started as an STNA and worked her way up to the Director of Nursing at a Skilled Nursing Facility. She made the transition to hospice a few years ago and loves the extra care she can provide to those during their end of life. Amber is a mom to an amazing 11-year-old boy. She loves to make decorated sugar cookies and loves the outdoors.

Harmony Semak Operations Manager For Harmony, long-term care isn’t just her career—it’s practically in her DNA. She grew up spending her childhood in nursing homes while her mom worked, and those early experiences shaped her passion for serving others, specifically the geriatric population. Over the past decade, she has built a career across a wide range of skilled nursing and long-term care facilities, earning a bachelor’s in social work and a master’s in healthcare administration along the way. It was during a hospice internship in undergrad, though, that she truly found her calling. Witnessing the interdisciplinary team care for the whole person—and their family—was nothing short of spectacular. That experience led her to a role as a bereavement coordinator, where she had the privilege of nurturing families through some of life’s most difficult moments. When she’s not working, she is chasing after her two boys, who make sure life is never boring! She recharges by going on nature walks or listening to a vast array of music. She’s always on the hunt for concert tickets for whoever’s coming through town.
